전 세계적으로 5억 3,700만 명이 당뇨병을 앓고 있으며 그 숫자는 점점 늘어나고 있다. 제1형 당뇨병에 걸리면 면역 체계가 인슐린을 만드는 췌장의 베타세포를 공격하고 파괴하며. 불행히도 췌장은 베타세포를 재생하지 못하기 때문에 제1형 당뇨병 환자는 평생 동안 인슐린 주사에 의존해야 한다. 이 과정은 기능적인 베타세포를 갖는 것만큼 정확하지 않으며 평생 인슐린에 의존하는 것은 엄청난 건강 및 경제적 부담을 수반한다. 그러나 지난 20년 동안 줄기 세포 연구와 치료법의 상당한 발전으로 제1형 당뇨병을 치료하는 데 필요한 새로운 인슐린 생성 세포를 만드는 유망한 방법이 밝혀졌으며, 과학자들은 줄기 세포를 사용하여 베타세포를 대체하고 새로운 유형의 치료법을 제공하기 위해 노력하고 있다.

이를 위해 줄기세포 연구자들은 인간의 베타세포 발달을 조절하는 특정 유전자를 결정하기 위해 많은 연구를 수행하였고, 지난 15년 동안 여러 연구 그룹이 이 정보를 독립적으로 사용하여 줄기세포에서 베타세포를 성공적으로 만들어 줄기세포 기반 제1형 당뇨병 치료를 위한 경로를 만들었다. 줄기세포 유래 베타세포를 당뇨병 쥐에 이식하면 당 대사를 조절하고 쥐의 당뇨병 상태를 개선할 수 있었고, 중요한 것은 생쥐가 인슐린 보충 없이 생존한다는 것이었으며, 이는 임상 시험의 단계를 설정하는 중요한 전임상 결과였다.

최근에는 미국 식품의약국(FDA)은 제1형 당뇨병 치료를 위한 만능 줄기세포 기반 요법의 첫 번째 임상 시험을 승인했다. 미국 하버드대학교의 더글라스 멜튼 (Douglas Melton) 전 ISSCR 회장과 Vertex Pharmaceuticals가 시작한 이 임상시험은 제1형 당뇨병 환자에게 줄기세포 유래 베타세포를 이식하는 것이 효과적이고 안전한지 테스트하는 것이었다. 2021년 10월, 이 임상 시험을 수행한 연구팀은 첫 번째 환자인 Brian Shelton의 초기 긍정적인 결과를 보고했다. 약 40년 동안 보조 인슐린에 의존한 후 Brian은 줄기세포 유래로 인슐린을 생산하는 베타세포를 한 번 이식한 지 90일 만에 자신의 인슐린을 생산할 수 있었고, 150일 후 Shelton은 주사하는 인슐린양을 92%까지 줄일 수 있어 인슐린 주사에 대한 의존도가 거의 사라졌음을 보여주었다.

또한 성숙한 베타세포 이식에 대한 대안적인 접근 방식으로, 다른 생명 공학 회사인 ViaCyte는 인체에서 성숙하고 기능하게 되는 줄기세포 유래 전구세포라고 불리는 것을 이식하였고, 이 전략의 독특한 측면은 미성숙 세포가 베타세포뿐만 아니라 당 대사를 조절하는 다른 세포 유형으로 발달할 수 있다는 것이었다. ViaCyte는 2021년 12월 임상 시험의 예비 결과를 발표했는데, 발표된 결과에 의하면 15명의 환자에 대해 보고했으며 이식된 줄기세포 유래 전구세포가 실제로 제1형 당뇨병 환자의 당 대사를 조절하는 세포로 성숙하는 것을 확인했다. 하지만 이러한 치료법이 임상시험 이외의 승인을 받기 전에 추가 연구가 필요하다.

더 많은 환자에게 이러한 치료법을 테스트하고, 가능한 장기적 효과를 조사하고, 환자의 면역 체계에 의한 공격으로부터 이식된 세포를 더 잘 보호하는 것이 중요하며, 줄기세포 연구에서의 이러한 성과는 우리를 제1형 당뇨병 치료제에 더 가까이 다가가게 하고 만성 질환을 앓고 있는 Brian과 같이 전 세계적으로 약 900만 명에게 혜택을 줄 수 있는 잠재력이 있는 중요한 이정표가 될 것으로 사료된다.

Clinicaltrials.gov에서 등록된 당뇨관련 질환 줄기세포 치료 임상연구 (Early phase 1 ~ phase 4) 중 중도포기와 시험단계가 알려지지 않은 연구를 제외하고 총 63개의 완료된 연구가 등록되어 있다.

이중 최근 10년인 2012년 1월 1일 이후로 현재까지 clinicaltrials.gov에 줄기세포를 활용한 등록된 완료된 임상연구는 총 30건으로 확인되었다.
